  1. Only a candidate who meets Scholarship criteria is required to complete this form.
  2. Must be Tanzanian
  3. Must be admitted to the UDSM
  4. The scholarship award is tenable for one academic year and renewable for successive academic years on condition that the recipient maintains a minimum GPA of 3.8.
  5. The information provided in this form is intended to help Scholarship Selection Committee understand the applicant’s academic and financial position for the purpose of assessing the applicant’s eligibility for scholarship award.
  6. This application form must be filled accurately and completely.
  7. Candidates must provide copies of the following mandatory documents as part of their application;
    • Copy of Birth certificate
    • Two essays explaining why you should be awarded with this scholarship and the reason of choosing your course of study
    • Admission letter from the University of Dar es Salaam.
    • Copies of academic certificates and transcripts
    • Copies of Death certificate(s) if parent(s) deceased
    • Copy of NIDA
  8. Candidates will be required to provide original copies of the required documents for verification within a given time when requested.
  9. Applicants who fail to submit the required documents will be discontinued/ disqualified from the selection process.
  10. The completion and submission of this form is not a guarantee of scholarship.
  11. Any false statements, omissions or forged documents will lead to automatic disqualification.
  12. Fondazone Edu & UDSM through selection committee, reserves the right to make the final determination of scholarship beneficiaries.
  13. Late and incomplete applications will not be considered for further processing.
  14. Ensure the completed form and supporting documents are submitted at DICA office UDSM, not later than time specified.
  15. Ensure you have not been awarded with a public loan or you have received a loan not exceeding the 30%
